This fall prevention focused assessment uses the evidence-based CDC STEADI framework (recommended by the American Physical Therapy Association) and other tools to give you a clear picture of your current fall risk and highlights the areas that can be improved.What’s Included:• Thorough balance, strength, gait & mobility testing• Complete home safety evaluation (lighting, rugs, bathroom, stairs, etc.)• Review of medications and other risk factors• Personalized recommendations with written summary report• Equipment and home modification guidance• Caregiver education and support

With over 20 years as a licensed Physical Therapist, I specialize in helping people stay safe and independent at home. I hold Master’s degrees in Kinesiology (Cal Poly SLO) and Physical Therapy (CSU Sacramento). My experience spans home health, acute care, and outpatient settings.
I take a practical, patient-centered approach — focusing on real understanding and sustainable independence.I reside in and serve the East Bay area (San Ramon, Walnut Creek, Danville, Pleasant Hill and surrounding areas).

Every comprehensive assessment utilizes standardized CDC STEADI frameworks (TUG, 4-Stage Balance, Chair Stand).• Closing the Loop: Following our assessment, you receive a concise, one-page clinical summary detailing objective balance scores, home hazards identified, and our plan if patient agreeable.• Resource Optimization: If a patient requires heavy, insurance-eligible skilled care, I will route them back to traditional home health. If they just need preventative positioning, I handle it directly.
2. For Assisted Living & Residential Care Communities (ALFs/RCFEs)
• Protect Length of Stay: I can help residents maintain their functional independence so they can age in place safely, protecting your community’s occupancy.• Reduce Liability Metrics: Proactive environmental safety checks and objective balance tracking drop facility fall rates.• Staff Support: I offer caregiver body-mechanics education and custom transfer recommendations to reduce staff/resident injury.
